This form is obtained by your accommodation provider - hotel or apartment landlord - when they register your arrival through the post office or local immigration department. The form will then be passed to you, and should be carried with your passport and visa during your stay. At the end of your time in Russia, or when you move to different accommodation, your accommodation provider will have to return your form to the authorities, so it must be kept safely. For this reason, you may be issued with a certified copy rather than the original document. You may be required to show this document if stopped by the police during your stay - to confirm that you are legitimately registered - but there is no longer any obligation to prove registration to immigration officials on your departure.
